Family Support Services encompass an array of programs that focus on the optimum quality of life for persons who have intellectual and developmental disabilities, as well as their families.
Home-Based Support Service matches each consumer with a self-directed assistant. The self-directed assistant will assess, review, and coordinate the individual’s personal, educational, and social needs to ensure that all are being met. The self-directed assistant is part of a team that works with the individual and the family to implement a Personal Support Plan (PSP) that can be adjusted and refined according to changing needs, goals, and wishes.
Intake Support Service offers access to information and referral to a variety of community resources and services. These resources and services may include legal, educational, health, and social needs for individuals with disabilities.
