Admissions Process

Come in for a tour!  Call us at 773-734-2222 to arrange a visit at either of our Community Day Service programs.  Meet with our staff, see the programs in action, ask questions.

Are you new to the process?  To receive services through South Chicago Parents & Friends, you need to begin with a referring agency.

If you live in Chicago, contact Community Service Options, Inc. (CSO) at (773) 471-4700 or email info@cso1.org.

If you live in the south suburbs, reach out to Suburban Access, Inc. at (708)799-9190.

Once you are working with either CSO or Suburban Access, you will have access to options including community day services, residential services, home-based programs, respite services, and links to various community resources.  You may request a referral to South Chicago Parents & Friends from them.
